File and Application for renew or duplicate PAN card the same way as you followed for getting for first PAN card.For this you have to submit your address proof, one new passport size photograph and duplicate form along with nominal fee charged for Card, Submit all these documents to your near local income tax office or its respective branch, & Do not forget to submit the xerox of FIR along with new application form, you will receive your duplicate PAN card withing a week.
